THE YOUTH WAS ADOPTED OR PLACED IN LEGAL GUARDIANSHIP AFTER AGE 15
THE YOUTH WAS REUNIFIED WITH THE BIOLOGICAL FAMILY AFTER AGE 14
THE YOUTH WAS IN FOSTER CARE AT AGE 18 EVEN IF THEY DO NOT ENTER INTO EXTENDED FOSTER CARE
Able to complete and clear a background check.
Ready to commit to educational and employment goals.
Willing to commit to FAHP participant agreement
